Policy: It is the policy of MOLARI HealthCare to comply with federal law regarding notice of privacy practices for our clients.
Purpose: To describe how medical information about clients may be used and disclosed and how clients can get access to this information.

MOLARI reserves the right to make changes to this notice and our privacy practices at any time and will provide clients with a revised copy prior to the next treatment date. Our current policy is listed below:
How we use and disclose your health information
The following categories describe different ways that we use and disclose medical information.
For Treatment - We may use and disclose medical information about you to provide care to you. We keep records of the care and services provided to you. Health providers use these records to deliver quality care to meet your needs. For example, we would disclose that a client has diabetes to the homemaker providing meal preparation for that client.
For Payment- We keep records of the care and services provided to you. For example, the Personal Care attendant servicing you will have you sign a timecard verifying the date, time and type of service or care provided. They will then present this to our billing department so that we may bill for those services.
For Health Care Operations - We use health information to improve the quality of our trained field and office staff, provide customer service and conduct business duties. For example, we use health information to train staff and evaluate the effectiveness of their care-giving ability during supervision of employees.
Sharing your Health information
There are limited situations when we are permitted or required to disclose health information without your signed authorization. These situations are:
Any other uses and disclosures, not described in this notice, require your authorization. You may revoke your authorization at any time with a written statement.
Please note that if the Massachusetts law requires more stringent guidelines for disclosure of Personal Health information, then we will follow the state law.

Complaints - If you believe that your privacy rights have been violated you have the right to file a complaint with MOLARI. Any complaints must be in writing and sent to MOLARI 's Privacy Officer, Lee Rudin RN, BSN at MOLARI HealthCare Services, 80 Center Street, Pittsfield , MA 01201.
