It is the policy of MOLARI HealthCare to comply

for our clients. Purpose: To describe how medical information about clients may be used and disclosed and how clients can get access to this information. - Maintaining the privacy of your health information - Providing this notice that describes the ways we may use and share your information - Following the terms of this notice MOLARI reserves the right to make changes to this notice and our privacy practices at any time and will provide clients with a revised copy prior to the next treatment date. A copy of our current policy will be available on our web site www.MOLARIinc.com. The following categories describe different ways that we use and disclose medical information. For Treatment. We may use and disclose medical information about you to provide care to you. We keep records of the care and services provided to you. Health providers use these records to deliver quality care to meet your needs. For example, we would disclose that a client has diabetes to the homemaker providing meal preparation for that client. information There are limited situations when we are permitted or required to disclose health information without your signed authorization. These situations are: - For public health purposes such as reporting communicable diseases or related illnesses - To protect victims of abuse, neglect, or domestic violence - For health oversight activities such as investigations and audits - When otherwise required by law - When requested by law enforcement as required by law or court order - To reduce or prevent a serious threat to public health and safety - If you are seeking compensation through workers compensation, we may disclose your protected health information to the extent necessary to comply with laws relating to workers compensation. This copy does not include information that was disclosed for treatment or payment purposes or for healthcare operations. - The right to request that we communicate with you concerning health matters in a certain manner. if you believe that your privacy rights have been
violated you have the right to file a complaint with MOLARI.
