How we use and disclose your health information.

The following categories describe different ways that we use and disclose medical information.

For Treatment. We may use and disclose medical information about you to provide care to you. We keep records of the care and services provided to you. Health providers use these records to deliver

quality care to meet your needs. For example, we would disclose that a client has diabetes to the homemaker providing meal preparation for that client.

For Payment. We keep records of the care and services provided to you. For example, the Personal Care attendant servicing you will have you sign a timecard verifying the date, time and type of service or care provided. They will then present this to our billing department so that we may bill for those services.

For Health Care Operations. We use health information to improve the quality of our trained field and office staff, provide customer service and conduct business duties.  For example, we use health information to train staff and evaluate the effectiveness of their care-giving ability during supervision of employees.
